This is a Cry of the Heart. It is the most universal human desire: to be partnered with a person who fulfills YOUR heart's desire and his too, so that the relationship is reciprocal. But your poem does not tell a story of that fulfillment but thoroughly details a crushing sense of loss. Its honesty cannot be disputed. This poem expresses a universal sympathy. It reminds me of something Beethoven wrote above the notes of his last string quartet: MUST IT BE SO? //YES, IT MUST BE SO.
